Disable booking address

Added Disable Address Fields on Booking Form (disableBookingAddress) 
setting to the package manager. It requires Credit Cards to also be 
disabled. This removes all address related fields and address validation 
from the booking process. The fields left will be limited to: First Name, 
Last Name, Email Address, Phone Number, Comments and accept disclaimer. 
This was implemented for the TLinks system but should work on any.
